Mission:
InnovAge Virginia PACE Richmond is dedicated to helping older adults live independently and with dignity in their communities through the Program of All-inclusive Care for the Elderly (PACE).
Services Provided
- Comprehensive Senior Healthcare: An expert interdisciplinary medical team provides coordinated care tailored to each participant's medical needs and social interests.
- Medical and Social Support: Nearly all medical care and social support are delivered at a center near the participant's home, serving as an alternative to nursing facilities.
- Specialty Care Access: Participants have access to dental, vision, hearing, and foot care services.
- Therapies: Occupational, speech, and physical therapy are available to support functional independence.
- In-Home Assistance: Support is provided in the home as needed, including personal care and assistance with daily activities.
- Medication and Equipment: Prescribed medications, medical equipment, and supplies are included in the program.
- Transportation: Wheelchair-accessible transportation is provided to and from the center and for medical appointments.
- Social Activities: A variety of social activities and support services are offered to promote engagement and well-being.
- Caregiver Respite: Respite services are available to support caregivers.
