Mission: ComForcare Companion Care Services aims to support homebound individuals by providing essential non-medical assistance to help them maintain independence and quality of life in their own homes.

Services Provided:

• Meal Preparation: Assistance with planning and preparing nutritious meals tailored to individual dietary needs.
• Medication Reminders: Ensuring clients take their medications on schedule as prescribed by their healthcare providers.
• Companionship: Providing social interaction and emotional support to reduce feelings of isolation and loneliness.
• Transportation: Helping clients get to appointments, run errands, or attend social activities safely.

Care Planning:

• A registered nurse develops an individualized care plan in consultation with the client, their family, and their physician for those requiring personal care.

Availability:

• Home Companions are available 24 hours a day, 365 days a year, offering flexible scheduling to meet the needs of clients and their families.
